Declaration of consent – Copenhagen Medical A/S

The legal foundation for the processing of your personal data is based on your consent, cf. General Data Protection Regulation, article 6, subsection 1, letter a.

For us to provide you with the best service and user experience, please read and accept the consent form below, granting us consent to storing your information in our user and patient record system. The period of storage depends on the type of data processed.

In MedicalMe, you may create a personal profile, whereby logging in with NemID or MitID, you gain access to several services provided by Copenhagen Medical A/S.

After you consent, you may

• book an appointment for Copenhagen Medical A/S’ healthcare services
• access services in Copenhagen Medical A/S’ health centers
• have your samples and tests handled and analyzed in our or external laboratories
• carry out and be guided during home tests
• access test and examination results, etc. in MedicalMe or via an access restricted web portal; this applies to both tests and examinations carried out in Copenhagen Medical A/S’ health centers and tests carried out at home
• display and download test certificates from MedicalMe, which Copenhagen Medical A/S collects with access to external storage
• with your phone´s camera, scan your personal QR-code, which opens a guide and registers both tests carried out in Copenhagen Medical A/S’ health centers and at home. The QR-code is unique and will not be stored on the phone
• receive calls or e-mails from Copenhagen Medical A/S with a view to receiving relevant information and guidance in connection with receiving one or more of our healthcare services
• receive calls or e-mails from Copenhagen Medical A/S with a view to improving our healthcare services and products

To gain access to the various healthcare services and/or our digital platforms, it is necessary for Copenhagen Medical A/S to obtain and securely store your personal data. According to the General Data Protection Regulation (GDPR) and the Data Protection Act, this data processing requires that you, as a recipient of our healthcare services, have granted consent to Copenhagen Medical A/S’ processing your personal data.

The following information is collected by Copenhagen Medical A/S in in relation to the usage of our healthcare services, appointment booking and use of MedicalMe:

• First name and surname
• Danish Social Security number
• Phone number
• Email
• Zip code
• Regular General Practitioners (GP) “ydernummer” (identification number)
• Your test and/or examination results, cf. the Danish Act regarding patient records (journalføringsbekendtgørelsen).
• Location, date and time of test or examination. This data is collected by Copenhagen Medical A/S in connection with statistics and record keeping
• Health information

 

By accepting this declaration of consent, you give Copenhagen Medical A/S permission to process your personal information given the above stated aim.

 

Feedback on Copenhagen Medical A/S’ products and services

Copenhagen Medical A/S aims to constantly improve our products and services as well as the overall user experience.

Therefore, when creating your health profile in MedicalMe, you automatically consent to Copenhagen Medical A/S using your contact information to contact you by telephone and to send out surveys concerning customer satisfaction by e-mail in order to improve the quality of our products and service.

Copenhagen Medical A/S logs and stores your feedback. We store this data for one year and it is only used for the above purposes, after which it is anonymized, whereby it will no longer be possible to identify you based on the answers you have given. The anonymized answers are used to develop and improve our services and products.

 

Withdrawal of your consent

Your consent for the processing of your personal data when performing healthcare services and using MedicalMe can be revoked at any time by deleting your account in MedicalMe.

When revoking the consent to process your personal data, we delete all the information about you that we are not obliged to store, cf. the Danish Act regarding patient records (journalføringsbekendtgørelsen).
